Step by Step:

2) Register your Username
3) Fill in your username, email, maxis reload pin number.

* Make sure you fill in correct data to us, so please double check before click submit button.

4) Enter Captcha (simple math to avoid spam) (exp: The sum of A and B is:?)
5) If you like to CC this form into your email, click “correct box”
6) click SUBMIT button.

Currently VIP Application Form temporary disable because we now looking for a new mail form. Current mail form do not support captcha to avoid nasty spam and malware. We will re-announce again if we successful setup a new form soon
